Understanding an Elevated PSA
What Is PSA?
PSA (Prostate Specific Antigen) is a protein produced by the prostate gland. While PSA is normal in small amounts, higher levels can occur for many reasons. Because many noncancerous conditions can raise PSA, an elevated PSA alone cannot diagnose prostate cancer.
What Happens If Your PSA Levels are Consistently Higher than Normal?
An elevated PSA level does not automatically mean you have prostate cancer. It can be related to other non-cancerous conditions. However, if PSA levels remain higher than normal for a period of months, the Florida Cancer Affiliates PSA Evaluation Clinic may recommend additional testing to determine if cancer has started to develop.
Before more tests are recommended, our physicians will evaluate:
Your PSA level and how it has changed over time
Your age and overall health
Family history of prostate cancer
Urinary symptoms such as frequent urination or the flow of urine slows or stops while urinating
Any previous prostate biopsy test results
Any prior imaging studies, including prostate MRI results, if available
Depending on your individual situation, our recommendations may include:
Repeat PSA testing to monitor whether it continues to rise
mpMRI - an MRI specific to the prostate
Specialized blood or urine biomarker testing to see if there are genetic changes related to cancer
A referral for a prostate biopsy, when appropriate, to have samples of prostate tissue examined for signs of cancer.
Not every patient requires every test. Your evaluation is personalized to your individual risk.

Will You Need a Prostate Biopsy?
If your evaluation suggests a significant risk of prostate cancer, a prostate biopsy may be recommended. A biopsy is the only way to determine whether cancer is present. If a biopsy is needed, we will coordinate a referral to a trusted local urologist if you do not already have one. Urologists specialize in performing prostate biopsies.
What Happens After the Biopsy?
After your prostate biopsy is completed, you will return to Florida Cancer Affiliates to review your results.
During your follow-up visit, we will:
Explain your biopsy findings in clear, understandable language.
Discuss whether cancer was found and, if so, its grade and risk level.
Review any additional testing that may be recommended.
Discuss all appropriate treatment options, including whether active surveillance is appropriate.
Answer your questions and help you make an informed decision about your care.
What Happens After a Prostate Cancer Diagnosis?
Not all prostate cancers require immediate treatment. Depending on your biopsy results and overall health, options may include:
Active surveillance
Radiation therapy
Surgery
Hormone therapy
Other advanced treatments, when appropriate
